The Waterloo Wellington Community Access Centre (CCAC)
The Waterloo Wellington CCAC coordinates health services/supports and aids admission into long-term care homes for residents of the Waterloo Region, Wellington County and the southern portion of Grey County. We work closely with people who are recovering from illness or injury, and with those who are living with chronic illness or disability. We help people avoid hospitalization, or return home quicker after being in hospital to recuperate in the comfort and privacy of their own homes.
The Waterloo Wellington CCAC is a dynamic, team-based organization that provides the highest quality of care, promotes ongoing learning, provides training, workshops and focus groups and has active staff participation in wellness and community-building activities and initiatives. We are fortunate to live and work in an area that boasts three world-class universities (University of Waterloo, University of Guelph and Wilfrid Laurier University) and the equally respected Conestoga College. Our local landscape includes both diverse urban areas and rural farming and Mennonite communities. We are located only one hour from a number of majors centres, including Toronto, London and the Niagara area, and well within driving distance of the Canada - US Border. Our region is also home to a number of annual festivals and events.
Acting as a vital link between clients and various health care practitioners, our staff collaborate with clients, families, caregivers or substitute decision makers, assisting them in achieving optimal health and independence. They are responsible for service planning and implementation, as well as enhancing the development of a client's sense of empowerment, supporting their ability to live independently and make their own informed health choices.
Although we often hire people for other administrative roles, the majority of our staff are Case Managers. We require that Case Managers have membership in good standing with the appropriate college, as a Registered Nurse, Occupational Therapist or Physiotherapist, with a degree at the Baccalaureate level, or membership in good standing with the College of Social Work, with a degree at the Master's level. One or more years of community case management experience is preferred, with proven skills in the areas of case management, assessment, communication, interviewing, problem solving, interpersonal communication and leadership, within a team-based setting. This position also requires proficiency in the use of a personal computer in a Windows networked environment and the ability to travel within the CCAC region.
