The Hamilton Niagara Haldimand Brant Community Access Centre (CCAC)

The Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB CCAC) is a leader in community health, linking people of all ages to vital home care services including case management, nursing, physiotherapy, social work, diet, occupational therapy, speech therapy and personal support. These services are planned and coordinated by our staff and delivered through local agencies at home or in a community setting. Our staff is also instrumental in connecting people with services such as meal programs, support groups and other resources. 

The HNHB CCAC staff plays a vital role by helping people in our community:

  • remain at home
  • avoid hospital admission
  • access support upon discharge from hospital and
  • explore long-term care options.

Our staff work with clients, their families and other health care providers including family doctors, hospitals, service agencies and long-term-care homes to provide seamless health care to people in the HNHB region. We are proud to be a Best Practice Spotlight Organization (BPSO) designated by the Registered Nurses Association of Ontario (RNAO).

  • Our service area extends over 7,000 sq. km and includes Hamilton, Niagara, Haldimand-Norfolk, Brant and Burlington.

As an organization built on teamwork, mentorship and a passion for quality care, the HNHB CCAC seeks applications from healthcare professionals who share our vision of providing outstanding care to every person we serve.

Case Managers lead the delivery of home care services. Case Managers are Registered Health Professionals with a B.Sc.N, B.Sc.PT, B.Sc.OT, M.Sc.SP, MSW, BSW or RN with another degree or diploma in a health related field. They also have additional training in case management.  Applicants should come equipped with: a minimum of two years recent experience in community health or a related field; knowledge of community resources and proven experience with decision making, collaboration and team-building.  Individuals must be currently licensed or registered according to the requirements of their profession in the province of Ontario. A car and valid driver's license are mandatory. Fluency in languages other than English would be an asset.

Our Case Managers are individuals who pride themselves on their professionalism and caring. Our Case Managers assess, plan, coordinate, implement and review clients’ care needs and match them to essential services provided by the CCAC or available in the community.
