The Central West CCAC serves clients within the northern portion of Peel region, all of Dufferin County, part of York region, and a small part of the city of Toronto. Approximately 773,000 people live in the areas we serve.

Our Case Manager positions require a B.Sc.N, B.Sc.PT, B.Sc.OT or MSW, combined with one or more years of community health or related clinical experience providing client-focused care; or a Nursing Diploma combined with ten years of community health or related clinical experience providing client-focused care. Licensure or registration according to the requirements of the profession in the province of Ontario, strong computer and decision-making skills, and the ability to work as part of an interdisciplinary team are required. A car and valid driver's licence are essential; the ability to speak a second language would be a definite asset. These are the minimum qualifications for the role.
