Toronto Central CCAC

The Toronto Central Community Care Access Centre (CCAC) is a bridge to community-based care for thousands of clients and their caregivers every day.  We serve people from all walks of life, and at each step in their journey. We provide care connection services including information and referral, care planning and access to home care services. For some clients, it is a brief relationship as they get back on their feet after an illness or hospital stay. For others, it is a long-term connection, providing ongoing support to manage chronic conditions, maintain their life at home, and stay active in their communities.

Everything we do at the Toronto Central CCAC is driven by one passion – helping people reach their best possible quality of life while working in partnership  with others to help make our health system the best it can possibly be. Our inspiration comes from the 1.5 million residents we serve in communities from the Toronto and East York areas, stretching from Lakeshore Blvd. W. to St. Clair Ave. E.

Our CCAC is part of a network of 14 CCACs across the province which has taken on a new leadership role as health system quality care connectors, supporting clients to the right place of care. We work as part of an inter-connected team, embracing our connections to community and social service agencies, and valuing the partnerships we have in place with service providers that help support and treat people at home and throughout the community.
Our Care Coordinators manage a varied and challenging caseload, ensuring that clients receive the information and the health and support services they need when they need them. They conduct comprehensive assessments of overall health status, needs, resources and eligibility and, when necessary, assist clients and their families in planning long-term care. Toronto Central CCAC embraces lifelong learning as a key factor in providing the highest possible quality of care to clients and offers its staff a wide range of educational and training opportunities to ensure this level of care is maintained.

Care Coordinator positions require a B.Sc.N (or Diploma), B.Sc.PT, B.Sc.OT, M.Sc.SP or MSW combined with one or more years of community health or related clinical experience providing client-focused care. Licensure or registration according to the requirements of the profession in the province of Ontario, strong computer and decision-making skills, and the ability to work as part of an interdisciplinary team are required. A car and valid driver's license are essential; the ability to speak a second language would be a definite asset. These are the minimum qualifications for the role.
