Central Community Care Access Centre
Located in the community of North York, South Simcoe County and York Region, our Central Community Care Access Centre (CCAC) serves a diverse client population, which includes residents of several ethnic groups.

Our Case Manager positions require a B.Sc.N, B.Sc.PT, B.Sc.OT, M.Sc.SP, MSW, BSW or RN with another degree or diploma in a health related field. Individuals must be currently licensed or registered according to the requirements of the profession in the province of Ontario and have two to three years of community health or related clinical experience. A car and valid driver's licence are mandatory. Fluency in languages other than English would be an asset. These are the minimum qualifications for the role.
