The Champlain Community Care Access Centre (CCAC) promotes personal health by simplifying access and managing care across the health system. Geographically, we are one of the largest CCACs, delivering care to over 20,000 clients every day. Our region includes Eastern Counties, Renfrew County, and a portion of Lanark, Leeds & Grenville, spanning a diverse demographic population of both rural and urban clients. With the head office location in our nation's capital, employment with the Champlain CCAC offers staff and their families a variety of choices with respect to community and lifestyle.

The Champlain CCAC provides access to a wide variety of quality home care services, specialized programs, referrals and education. We help clients navigate comfortably through a complex health care system to ensure the highest quality of care, and are committed to making the best use of home and community care resources.

Acting as a vital link between clients and various health care practitioners, our Case Managers collaborate with clients, families, caregivers or substitute decision makers, assisting them in achieving optimal health and independence. They are responsible for service planning and implementation, as well as enhancing the development of a client's sense of empowerment, working in partnership with them and supporting their ability to live independently and make their own informed health choices.

As an organization undergoing significant growth, the Champlain CCAC now has various full-time, part-time, and casual opportunities available. In addition to positions in our core service area of Case Management, the Champlain CCAC offers various other exciting career options including Client Services, Quality, Health Administration, Administrative Services (e.g. Finance, HR, IT and Communications), as well as Clerical and Administrative Support.
