Community Care Access Centres connect people with the care they need,
at home and in their communities
CCACs help people find their way through Ontario’s health care system, understand their options and connect them to quality community based health care and resources.
Assisting people of all ages and cultures, whatever the diagnosis, CCACs plan, coordinate and ensure the delivery of a full-range of in-home and community-based services to support clients and their caregivers. We arrange for quality health-care professionals – nurses, physiotherapists, social workers, registered dietitians, occupational therapists, speech therapists and personal support workers – to provide a range of care and supportive services to assist people at home and allow them to enjoy the best possible quality of life.

As part of the current health care transformation across the Province, Local Health Integration Networks (LHINS) are a recent addition to our local health care systems. CCACs across the province are aligned to the geographic boundaries of the LHINS, based on the belief that community-based care is best planned, coordinated and funded in an integrated way at all levels.
In total, there are 14 CCACs in communities across Ontario that are funded by Local Health Integration Networks through the Ministry of Health and Long-Term Care. This means that CCAC advice and services are covered by OHIP.
